Main duties of the job:
  • To work within the Care Group to provide, facilitate and further develop a comprehensive patient-centred, nurse-led post-transplant follow-up service which includes patients who have undergone kidney transplants, simultaneous kidney/pancreas transplants and simultaneous liver/kidney transplants.
  • To be responsible for the day-to-day management of the nursing and administrative service in the department.
  • To autonomously provide high quality nursing care in a minimum of 3 clinics per week.
  • To take a leading role in the strategic development and management of the services provided to transplanted patients so that outcomes meet recognised national standards.
  • To ensure appropriate and effective utilization of human and physical resources.
  • To effectively lead, motivate and be a role model for staff.
  • To take a lead role in research, audit and measuring quality indicators within the specialty.
  • To develop all staff within the multidisciplinary team, providing support, advice, formal and informal education.

Main Duties and Responsibilities:
  • To co-ordinate the care of transplanted patients ensuring that patients are closely monitored and effectively supported to optimise their transplant function and quality of life.
  • To use advanced clinical assessment skills and judgment to assess, plan, implement and evaluate care for transplanted patients with complex physical and psychological problems using a recognised evidence-based model of care.
  • Monitor and make treatment interventions in order to manage symptoms associated with CKD and transplantation. If qualified as an independent prescriber, prescribe medications as clinically indicated.
  • To order, interpret and act on relevant tests and investigations.
  • To provide timely and appropriate education and health promotion information to patients, including on treatment options for renal replacement therapy and supportive management for those with failing transplants.
  • To manage the work-up for re-transplantation of suitable people with failing transplants, ensuring they have adequate knowledge and are both physically and mentally prepared for re-transplantation and activated pre-emptively whenever possible.
  • To practice within the Renal Unit philosophy and develop clinical practice within the framework of the Trust’s Nursing/Midwifery Strategy.
  • To ensure good communication occurs with external and internal agencies caring for the patient.
  • To coordinate and develop the peer support service.
  • To discuss referral to GPs, community nurses and other medical specialities as necessary with appropriate clinician, taking responsibility for acting as a central liaison point for the above.
Clinical Governance:
  • To ensure data collection is accurate and up-to-date. Ensure renal database reflects clinic work.
  • To assess the desired level of clinical performance and be responsible for ensuring these are achieved within the relevant clinical setting.
  • To ensure that the transplant clinic is involved in all Unit quality initiatives and represented on relevant working groups within the Unit.
  • To ensure high standards are achieved in line with NICE, Renal Association and all other related clinical practice guidelines.
  • To participate in the Trust’s patient satisfaction surveys and audits.
  • To actively participate in the audit and evaluation of the clinics ensuring the service is modelled according to the needs of the client.
  • To monitor patient waiting times and ensure that these are within keeping with the Patients Charter.
  • To participate in the development and induction of clinical guidelines relevant to transplant services.
People Management and Performance:
  • Lead, coach and manage the performance of the team in line with good people management practices, ensuring excellence is recognised and underperformance is addressed.
  • Participate in regular performance appraisal meetings and ensure each member of the team has a clear set of objectives and development plans.
  • Ensure the team is compliant and up-to-date with all statutory mandatory training together with any professional training requirements.
  • Manage team absences including sickness in line with Trust policy ensuring the appropriate return to work meetings occur, e-roster is updated and productivity is kept to the highest possible level.
  • Identify and fill any vacancies that arise within the team in line with the Trust’s recruitment policy and process.
  • Identify talent and support the internal talent management process in order to attract, retain and succession plan for your team.
  • Review skills mix at regular intervals in order to identify any potential opportunities to maximise resource utilisation/allocation, ensuring job descriptions are kept up-to-date.
  • Ensure overall wellbeing of the team is maintained. Continuously support in improving the morale of the team and implementing a culture of zero-tolerance for bullying and harassment.
Clinical Management:
  • To co-ordinate patient activity within the clinical area, maintaining effective liaison with all other clinical areas in the department and all other support personnel.
  • To develop the transplant service in line with the needs of the patients using past experience, research and current demands to plan the service for the future.
  • To accept responsibility for the monitoring, ordering and maintenance of MSSE and pharmacy used with the area.
  • To establish policies and procedures for the services offered within the clinical area, working in close liaison with senior colleagues.
  • To ensure regular and effective communication systems are implemented and utilized across the MDT.
  • To monitor the nursing budget within the area, reporting any unexpected expenditure and costs incurred and ensuring that nursing and physical resources are used effectively and efficiently.
  • To manage clinic templates and numbers of both patients and multi-disciplinary staff to ensure all clinics run smoothly and efficiently.
  • To contribute to the development of nursing practice with the Care Group and across the Trust.
  • To actively involve service users in providing feedback or their experience of the service and suggestions for improvement.
  • To optimise patient safety ensuring formal reporting and recording of adverse incidents and dissemination of learning from these events in line with Trust policy.
  • To participate in responding to and learning from complaints.
  • To work with the Human Resources Manager on the resolution of complex HR issues.
  • To discuss with the Matron and Head of Nursing any plans in relation to service development, service improvement and the resolution of any ongoing problems.
  • To “act up” across suitable roles as required by senior management team according to service needs, or as part of an agreed personal development plan.
  • To support nursing colleagues with delivery of renal services outside of the transplant clinic when required.
Education and Research:
  • To facilitate education and learning within the clinical area, to benefit nursing staff, renal course students, medical colleagues, support staff, patients and relatives.
  • To provide formal and informal teaching relating to transplantation to colleagues as required.
  • To participate in preparation of abstracts and delivery of presentations for conference.
  • To maintain up-to-date GCP training and provide early education to patients about local research trials and work closely with research team to offer maximum number of patients opportunity to participate in research.
  • To maintain an up to date library (physical and electronic as appropriate) of information and resources in clinical setting informing staff, patients and relatives about Renal Replacement Therapy services available within the Renal Unit.
  • To facilitate research and innovation within the area, acting as a link nurse.

  • Knowledge and experience of caring for kidney transplant patients in a specialised Kidney transplant setting
  • Knowledge of national renal transplant policies and bodies, including NHSBT, NSF, NICE, The Renal Registry, and their impact on this role.
  • Ability to work independently and as part of a team
