Job overview We are seeking an experienced nurse capable of taking on a dual role. Provide nursing support to our expert team of clinical staff during day shifts as part of our dynamic, highly reputable multidisciplinary team. Collaborate with Senior Discharge Planners, Matrons, Lead Nurse, and Medical Team to manage the Discharge Planning & In-Reach service on the unit. As a Junior Sister, you should hold neonatal and mentorship qualifications and have some previous experience at band 6. You will receive support in developing management skills through personalized professional leadership and clinical development. Main duties of the job Coordinate the In-reach Clinic, working closely with senior Neonatal Discharge Planners and community services to actively reduce length of stay. Partner with the multidisciplinary team and senior discharge planners to determine the best ongoing care for each baby, whether staying on the unit, transferring to a local hospital, going home, or moving to a pediatric ward. Work closely with Senior Nurses and Discharge Planners to manage patient flow from admission to discharge while maintaining high-quality patient care and resource efficiency.
