The predicted salary is between 50000 - 70000 £ per year.

Following the release of ARRS GP information and funding, we are looking for newly qualified (within 2 years and not having a substantive post), enthusiastic forward-thinking GP(s) to join Kilburn Partnership PCN on a fixed-term salaried contract, preferably with an immediate start date.

The postholder’s role will primarily involve patient-facing clinical work supporting the PCN GP practices alongside a focus on PCN initiatives including novel, innovative at-scale work and opportunities. Additionally, they will offer support to the GP Enhanced Access Hub and care homes within the PCN, providing enhanced services to the registered population.

Main duties of the job

Main Duties

  1. Assess and triage patients, including same-day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  2. Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  3. Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement, and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate.
  4. Deliver high-quality healthcare to care home residents who often have complex medical needs, conducting thorough initial assessments upon admission to the care home.
  5. Use complex decision making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
  6. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
  7. Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health. Implement and participate in vaccination and immunisation programmes for both adults and children.
  8. Support the delivery of advanced care plans.
  9. Actively take a personalised care approach and population-centred care approach to enable shared decision making with the presenting person.
  10. Recognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health records.
  11. Be aware of your duties and responsibilities regarding current legislation and adhere to our policies and procedures on Safeguarding Children and Safeguarding Adults.

Delivering a quality service

  1. Recognise and work within own competence and professional code of conduct as regulated by the GMC.
  2. Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures.
  3. Prioritise, organise, and manage own workload in a manner that maintains and promotes quality.
  4. Deliver care according to CQC, NSF, NICE guidelines, and evidence-based care.
  5. Evaluate patients’ response to health care provision and the effectiveness of care.
  6. Supervise other HCPs including ARRS staff and help support their training.

Our PCN consists of four GP practices caring for a population of just under 30,000 patients.

We work together with a range of local providers, including acute trusts, community services, social care, voluntary sector and patient participation groups, to offer more personalised, co-ordinated health and social care to our local population.

Qualifications

Essential

  • Newly qualified GP, within 2 years and not having had a substantive post.
  • Registered with the General Medical Council (GMC) and license to practice.
  • Membership of appropriate Medical Organisation (MDU, MPS, etc).
  • Inclusion in NHS England / NW London Performers List of Medical Practitioners.
  • Annual appraisal updates.
  • Experience of working in a primary care environment.
  • Experience of dealing with a range of clinical conditions.
