About Cornwall Supported Living: Our specialist Supported Living and Outreach service provides support for people with Learning Disabilities, Autism, Mental Health needs and Acquired Brain Injuries. We understand the importance of providing person-centred support that enables independence and meets each individual's needs.

Responsibilities:

  • Managing a team of Support Workers
  • Providing high standards of care and support for people we support, in line with care and support plans
  • Encourage good community relationships by promoting a variety of activities
  • Accurately report, record and maintain records as required and working to care plans
  • Liaise with health professionals and their families
  • Complete supervisions and audits
  • Ensure that all duties undertaken by reporting staff are performed to required standards, contributing positively to provide a happy, efficient and homely atmosphere within the Home.
