Responsibilities
- Deliver high-quality, safe, and person-centred support in person home and during office-based shifts, working on a rota over seven days.
- Act as part of the contingency plan to cover absences and holidays within the package you manage.
- Safeguard the well-being of the people you support at all times.
- Build professional and supportive relationships with the people you support and their chosen contacts.
- Identify achievable support and rehabilitation goals.
- Lead and guide Support Workers to follow and implement care plans.
- Agree support schedules with commissioners, the people we support, and Support Workers, ensuring funded hours are delivered as agreed.
- Keep the Case Manager informed of support delivery, achievements, and any issues.
Line Management
- Line manage Support Workers, including regular supervisions and observations.
- Provide guidance, training, and mentorship to team members.
- Lead by example and maintain open, clear communication with all team members.
- Work with the Recruitment team to hire Support Workers according to individual needs and preferences, and ensure they are fully briefed and trained.

- Experience in providing support to individuals with physical disabilities, following their bespoke care plans and showcasing an understanding of their unique needs and challenges.
- Competence in personal care tasks, with a focus on dignity, comfort, and well-being.
- A positive and professional attitude, creating an encouraging environment for those you support.
- The ability to inspire and motivate individuals to work towards their goals and aspirations.
Desirable Skills & Experience
Experience of working with individuals with complex nursing needs.
