As a Move on Officer, you’ll work with people within the city who have been living in Support Housing and are now ready to take that step into independent living. You will do this by working with Supported Housing providers and the local authority to help deliver a move‑on plan for people who are ready to transition into sustainable independent living.
Job Requirements
- Conduct assessments, risk assessments, inductions, and support plans collaboratively, reviewing plans monthly.
- Develop and deliver person‑centred, trauma‑informed move‑on plans tailored to individual needs, strengths, and goals.
- Support financial readiness and explore suitable housing options for successful move‑on.
- Provide resettlement support for up to 8 weeks following a successful move into a new tenancy, and up to 10 weeks of light‑touch tenancy sustainment support.
- Empower customers to maintain independence, manage their homes, and access essential items through local partnerships.
- Complete timely administration, monitor caseloads, and contribute to performance reporting.
What We Are Looking For
- Excellent communication skills.
- Understanding of great customer service and customer care.
- The ability to work under pressure and manage multiple tasks and demands.
- The ability to work with relevant departments and partners across this city and liaise with statutory services where appropriate.
- Understand the principles of Value for Money.
- Full UK driving licence and use of your own vehicle for work.
- DBS check required.
