We are seeking an experienced and motivated Paramedic Practitioner or Advanced Nurse Practitioner to join our Primary Care Network in an ARRS funded role, based at St Agnes Surgery. The post-holder will work autonomously within a supportive GP practice environment while being fully integrated into the wider PCN multidisciplinary team.
The role supports PCN DES objectives by improving access to care and reducing GP workload, while delivering high quality, patient‑centred care. The role includes:
- Undertaking home visits
- Managing urgent and same‑day demand
- Supporting patients with acute and long‑term conditions
The post-holder will also be responsible for managing their own clinical workload, including referrals and results.
