Summary
Do you want to work within an innovative forward‑thinking PCN?
We are seeking a senior mental health practitioner with leadership experience to join us as the professional lead for our Mental Health & Wellbeing team.
The role is band 7/8a equivalent, depending on experience.
The successful candidate will be a qualified nurse, with the relevant professional registration (NMC).
They will be a prescriber or willing to work towards this once in post and will be employed directly by the PCN.
Key Responsibilities
You will be responsible for providing clinical leadership for our Mental Health & Wellbeing team, which spans both practices within our PCN. The role involves:
- Providing clinical leadership and management of the Mental Health & Wellbeing team.
- Participating in pre‑referral discussions with secondary care and contributing to transformation work.
- Developing and maintaining partnerships with local services, including community health & wellbeing workers, MHBATT, and neighbourhood teams.
- Leading the PCN social prescribing team as part of the mental health and wellbeing provision.
- Supporting colleagues to deliver an excellent first primary care contact for patients with poor mental health.
- Playing a key role in extending access to care and addressing health inequalities within the local population.
Qualifications & Experience
You should have
- Qualified nurse with NMC registration.
- Leadership experience within primary care or another healthcare setting.
- Experience delivering mental health services, preferably within a primary care context.
- Prescriber status or commitment to becoming a prescriber within the role.
- Excellent communication and partnership‑building skills.
- Working Hours
Part‑time, full‑time, and job‑share applications will be considered, but the position requires at least three days a week.
