East Cornwall PCN is seeking an experienced Frailty Practitioner to support our reactive frailty service. The role involves patient-facing work across Saltash/Torpoint and Liskeard/Looe areas, with a hybrid pattern between home, GP practices and other health locations.
Regular travel within the locality is required. You will assess, intervene and coordinate care for people living with frailty, aiming to prevent hospital admissions and support safe return home after discharge.
