Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team. We are proud to have recently been rated ‘Good’ by the CQC and remain committed to continuous improvement as we further develop and enhance our services.
The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities – helping patients live well at home, in care homes, or in other community settings.
Given that a significant proportion of our population is older, this role will include a focus on supporting frail and elderly patients, while also addressing the broader needs of adults of working age with complex or chronic conditions.
The Community Matron will play a central role in:
- Enabling patients to remain independent and at home for as long as possible
- Reducing avoidable hospital admissions
- Coordinating care across primary, community, and social care services
This position is ideal for a clinician who is passionate about proactive, patient-centred care, enjoys working across a diverse community, and wants to make a meaningful difference in the lives of both older adults and younger adults living with complex health needs.
