Location: Community-based – Rosedean House Surgery & patient homes
Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team. The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities — helping patients live well at home, in care homes, or in other community settings. Given that a significant proportion of our population is older, this role will focus on supporting frail and elderly patients, while also addressing the broader needs of adults of working age with complex or chronic conditions.
Key Responsibilities
- Enabling patients to remain independent and at home for as long as possible
- Reducing avoidable hospital admissions
- Coordinating care across primary, community, and social care services
