Responsibilities:

  • Conduct thorough holistic assessments of individuals' mental and physical health needs, social circumstances, and support systems.
  • Develop, implement, and evaluate personalised care plans in collaboration with service users and the multidisciplinary team.
  • Coordinate and deliver a range of therapeutic interventions, medication management, and practical support.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed; arrange, contribute to, and document care reviews including telephone, home visits, video and face-to-face reviews as appropriate within the care group SOP.
  • Supervise junior staff members and support students in placement.
  • Work collaboratively with the multidisciplinary team, patients, carers, family members and internal and external agencies.
  • Use effective communication skills and demonstrate self-governance.
  • Show knowledge of high-level interventions and community assessment skills.

Qualifications & Experience:

Existing qualifications or a willingness to undertake training relevant to the post is expected.

Location and Salary:

The high-cost area (HCA) allowance, also referred to as London Weighting, is a payment made to employees who work in London and the surrounding areas. As this post is based in Dartford, Gravesham or Swanley, the successful applicant will receive an additional payment each month. Salary: 5% of basic salary, subject to a minimum payment of £1,303 per year and a maximum payment of £2,198 per year (2025 figures). The agreed annual amount will be divided into 12 equal amounts and paid monthly.
