The main elements of community mental health support work focus on collaborative working with service users to promote engagement, facilitate independence, and self‑management. Our expectation is that you will establish therapeutic relationships to address recovery goals and help service users access resources and services for involvement in the local community.

As part of your role you will also provide support to nursing staff with Physical Health Clinics, assist the multidisciplinary team (MDT) with monitoring treatment plans, and visit service users in their homes as required. You will be part of the secondary mental health services team, focusing on the direct needs of users with learning disabilities and working across boundaries of care, organisations and roles.

You will give time to an allocated group of users to promote their recovery and help maintain them in the community, building their support network. You will support users to identify needs, and assist care coordinators to plan, implement and evaluate individual care plans, using the recovery model to focus on each person’s goals. Your day‑to‑day work will be underpinned by agreed values and skills, and you will actively participate in delivering interventions with guidance from the MDT.

Qualifications

  • Full, valid UK driving licence and access to a car/vehicle for business purposes (unless you have a disability as defined by the Equality Act 2010).
