Overview

We are looking for an empathetic, resilient, and highly organised Social Work Assistant to join Dartford CRHT.

The role supports the Trust values of being Respectful, Open, and Empowering while delivering social work interventions during crisis.

You will work under the guidance of the Social Work Team and with the MDT (nurses, occupational therapists, doctors, and support workers) to help service users and their carers navigate crisis recovery.

This is a fast-paced, community-facing role that bridges social care with clinical crisis intervention.

Responsibilities

  • Undertake specified tasks with service users as agreed with the Social Worker/MDT to support safe and effective crisis care.
  • Liaise with partnership agencies, statutorily and voluntarily, on behalf of service users and the wider MDT.
  • Encourage feedback from service users, carers, and other professionals about service delivery.
  • Be aware of safeguarding needs and report issues promptly; complete necessary paperwork.
  • Contribute to safeguarding investigations.
  • Contribute to risk assessment and risk management planning.
  • Support the Child Protection policy in completing and recording contracts with SSD and Persons with PR.
  • Support capacity assessments where appropriate and when advised by the team.
  • Develop skills to work effectively with service users experiencing severe and enduring mental health problems, including crisis situations.
  • Stay informed about changes in legislation, policy, and practice guidance.

Qualifications
