An exciting opportunity has become available for a Band 7 Senior Practitioner to take on the clinical leadership responsibility for the Crisis Home Treatment and Rapid Response Team 24/7 to a team of clinicians from different professions. If successful, you will ensure a high-quality service is provided and care is delivered to all service users on the HTT and RR caseload who are experiencing a Mental Health Crisis by the Trust's policies and procedures and the care programme approach.
The post holder must hold a full, valid UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Responsibilities- Contribute to the implementation of clinical and interdisciplinary strategies to ensure the delivery of a high standard of care and service objectives and to the overall corporate objectives of the organisation.
- Work as a member of the Crisis Home Treatment Team, ensuring team clinical performance is optimised, meeting agreed KPIs, and ensuring that CRHT is a responsive service providing timely assessment and offering a range of home treatment interventions as an alternative to hospital admission.
- Have robust working relationships with colleagues within the acute and community settings; liaison psychiatry, place of safety, and inpatient teams, so seamless acute care is provided for service users.
