The role provides mental health support to Ashford Primary Care Network (PCN) in Kent. The practitioner will be embedded as part of the PCN Multi‑Disciplinary Team and work closely with the community mental health MDT, acting as a bridge between primary care and secondary mental health services by delivering high‑quality mental health care in primary care settings.
Key Responsibilities
- Provide mental health support and advice to members of the Ashford PCN.
- Collaborate with community mental health MDT and secondary services to ensure seamless care.
- Act as gatekeeper and guardian for referrals to secondary care services.
- Deliver high‑quality interventions within primary care settings.
- Work autonomously and creatively in a busy, demanding role.
Qualifications and Experience
- Experienced Mental Health Practitioner – Nurses, Social Workers or Occupational Therapists.
- Highly competent with excellent assessment skills and knowledge of local resources.
- Ability to work independently within a multidisciplinary environment.
Employment Details
- Hours: 2 × 22.5 hours per week (total 45 hours).
- Work as part of the Integrated Teams in GP surgeries within Ashford PCN, as part of the Additional Roles Reimbursement Scheme (ARRS).
