Overview

The role is a Mental Health Practitioner position within the Tunbridge Wells Primary Care Network (PCN) in Kent. The practitioner will be embedded as part of the PCN Multi‑Disciplinary Team, working closely with the community mental health MDT and drawing on secondary mental health services to provide a bridge between primary and secondary care.

Responsibilities

  • Provide high quality mental health assessment and intervention within primary care settings.
  • Work autonomously and creatively in a busy, demanding role, acting as a gatekeeper and guardian for referral to secondary care services.
  • Collaborate with GP surgeries and the integrated team to deliver services across the community.
  • Access and utilize local mental health resources and secondary services to support patients.

Qualifications

  • Experience as a Mental Health Practitioner (Nurse, Social Worker, or Occupational Therapist).
  • Full, valid UK driving licence with access to a car or vehicle for business purposes (unless a disability as defined by the Equality Act 2010 applies).
  • Excellent assessment skills with knowledge of local resources.
  • Proficiency in delivering interventions within a community or primary care setting.
