Working 16 hours per week, you will play a key role in delivering high-quality nursing services and supporting the health and wellbeing of our practice population.

Your responsibilities will include:

  • Delivering evidence-based nursing care within the practice setting.
  • Managing and supporting patients with long-term conditions including diabetes, asthma, COPD, hypertension, and cardiovascular disease.
  • Undertaking chronic disease reviews.
  • Providing cervical screening, immunisations, vaccinations, travel health advice, and wound care services.
  • Supporting patients with lifestyle changes and delivering health promotion and wellbeing initiatives.
  • Identifying patients at risk of developing long-term conditions and providing proactive interventions.
  • Supporting patients with medication management and self-care.
  • Maintaining accurate clinical records and contributing to quality improvement programmes.
  • Working collaboratively with the wider multidisciplinary team and external healthcare professionals to ensure coordinated patient care.
  • Supporting safeguarding, infection prevention, and clinical governance activities within the practice.
