We are a GP Surgery, looking to hire a full time Pharmacy Technician to manage and coordinate repeat prescriptions and prescriptions related support for community and care home patients.

The Pharmacy Technician will support the GP practices to deliver medicines optimisation and reconciliation from hospital discharge and other secondary care communications, liaising with community pharmacies looking for available alternative medications in the event of medication shortages.

You will be working with the team to ensure medication changes are safe and timely managed with patient counselling where appropriate. The post holder will work within their own competency under the supervision of our clinical team.

The post holder will support the pharmacy team with specific quality improvement projects in relation to medicines, managing prescriptions, clinical auditing and, where appropriate, giving advice to both patients and members of the primary care team.

You will support practice reception teams in streaming general prescription requests including care home prescription queries, so as to allow GPs and clinical practitioners to review the more clinically complex requests.

You will be sharing jobs like coordinating appointments for care home patients and dealing with queries from care home staff, identifying the support needs from emails, hospital letters, in-house correspondence etc. and the options to address these and to organise it appropriately, checking and recording results (blood pressure, ECG, phlebotomy), filing test results, updating patient records etc.

You will be organising and conducting appropriate actions for patients, assigned by our clinicians or action from the hospital letters with a holistic approach.
