Job Purpose
Provide autonomous nursing care while independently prescribing medicines within scope of practice for patients with complex needs in the community.
Key Responsibilities
- Assess, diagnose and manage common conditions.
- Independently prescribe and review medicines (V300).
- Optimise medicines and undertake medication reviews.
- Coordinate multidisciplinary care.
- Support admission avoidance and long-term condition management.
- Maintain prescribing governance and clinical records.
- Maintain Accurate same-day coding/KMCR documentation
