Deliver person-centred nursing care for patients with complex physical, mental health and social care needs within the Single Neighbourhood Team.
Key Responsibilities
- Maintain Accurate same-day coding/KMCR documentation
- Holistic assessment and care planning.
- Deliver evidence-based nursing interventions.
- Manage long-term conditions, frailty and palliative care needs.
- Coordinate multidisciplinary care.
- Promote self-management and health education.
