Deliver person-centered nursing care for patients with complex physical, mental health and social care needs within the Single Neighbourhood Team.
Key Responsibilities
Maintain Accurate same-day coding/KMCR documentation
Holistic assessment and care planning.
Deliver evidence-based nursing interventions.
Manage long-term conditions, frailty and palliative care needs.
Coordinate multidisciplinary care.
Promote self-management and health education.
Maintain accurate documentation and governance.
