Dartford MODEL PCN is seeking a Frailty Nurse to deliver proactive, person-centred care across GP practices, patients' homes, and care homes. You will conduct CGAs, coordinate care, and promote anticipatory planning for adults with frailty and complex health needs.
Operating within an MDT, you will support housebound patients, care home residents and those identified through frailty registers, aiming to prevent unnecessary hospital visits.
