Provides integrated support to patients and physicians by providing a \’hands-on\’ approach to total quality patient care through the delivery of a full continuum of medication management services, including preparing medications, providing pharmacological information, monitoring patient drug therapies, and supporting medication adherence.

Responsibilities

  • Prepares and dispenses medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
  • Provides thorough patient consultations on disease state management and medication therapies.
  • Determines clinical appropriateness for immunizations; counsels and provides immunizations as indicated.
  • Manages medication therapy, including reviewing and monitoring medication regimens, advising clinicians on evidence-based therapy, and documenting interventions.
  • Completes pharmacy operational requirements by organizing and directing technicians\’ workflow; verifying their preparation and labeling of pharmaceuticals; verifying prescriptions; coordinating care; resolving insurance issues; and identifying opportunities for financial assistance.
  • Supports 340B mission and compliance; possesses sufficient 340B knowledge to audit claims and assist during external audits.
  • Ensures compliance with state and federal drug laws; maintains records for controlled substances; removes outdated or damaged drugs; supervises support personnel; stays informed of legislation; and advises management accordingly.
  • Refers patients to other health professionals as necessary.
  • Develops knowledge through education; trains pharmacy staff, students, interns, externs, residents, and healthcare professionals.
  • Organizes and updates data using database applications.
  • Implements observations and recommendations to improve operations, productivity, quality, and customer service.
  • Identifies trends, resolves problems, and recommends improvements.
  • Participates actively in process improvement initiatives.
  • Maintains confidentiality of organizational information.
  • Enhances professional knowledge through education, reading, networking, and participation in organizations.
  • Willing to undertake additional duties for organizational betterment.
  • Performs other duties as assigned.

Job Qualifications and Specifications

Required Licensure/Certification: Must hold a Doctor of Pharmacy (PharmD) or BS in Pharmacy and be licensed as a Pharmacist in Rhode Island.

Years of Experience: Minimum 5+ years, with preferred 8+ years in a specialty or retail pharmacy setting.

Advanced Education: Preferred certifications include: Certified Specialty Pharmacist (CSP), immunization, medication therapy management, diabetic educator, cardiovascular educator, 340B ACE.
