Role Responsibilities

We are recruiting Social Care Officers with a range of skills and experience in the North Kent Adult Social Care Connect Team.

Full-time and part-time opportunities are available.

We operate in a hybrid model of office-based and remote working to meet service needs.

About Us

This is an exciting opportunity to join our Adult Social Care Connect Team across Kent, focusing on locality working, person-centered care, and good practice.

We work with adults with various care needs, placing individuals at the center of their support. Responsibilities include initial enquiries, relationship building with individuals, carers, and partners, crisis intervention, and assessments under the Mental Capacity Act. Supervision is provided by an experienced registered practitioner.

Our interventions are based on excellent practice, innovation, and meaningful measures, emphasizing relationship building with individuals, families, and communities to empower local support options.

About You

You should have experience working with people with social care needs and conducting initial assessments.

You must also possess:

• Experience in a multi-agency environment
• Good computer literacy and report writing skills
• Excellent interpersonal, mediation, and negotiation skills
• Ability to work collaboratively with individuals and partners to identify community resources
