You will support adults with mental health needs, learning disabilities and neurodiversity, beginning enablement in hospital and continuing after discharge into the community across Kent.
You will empower individuals to regain independent living skills, work with hospital and community teams, and travel across Kent to meet people's changing needs and priorities.
