The Homes not Hospitals Team works to discharge plan from hospital for people with a Learning Disability and/or Autism who have been identified as part of the Homes Not Hospitals cohort. We are looking for a KSG Social Worker to fill a permanent part-time post (22.2 hours per week). You will play a key role in delivering the local authority’s statutory duties, including:

  • Assessing eligible social care needs
  • Undertaking Mental Capacity assessments
  • Completing community DoLS applications
  • Assessing Section 117 needs
  • Acting as social supervisor where necessary (training will be given)

Utilising strengths and rights-based approaches, you will support people to reintegrate into their communities, reduce inequalities, improve quality of life, instil hope and realise real change. You will have experience of working with complexity, diversity and of assessing and mitigating risk and of working alongside health and third sector partners to develop robust aftercare plans. We are particularly interested in attracting applications from people who have experience/expertise in working with people who have a learning disability or autism but we welcome applications from across social care.

You will manage a caseload, acting as social supervisor for Conditionally Discharged persons when required, and will regularly review and evaluate social care interventions and services to ensure needs continue to be met in the most appropriate way. You will also act as supervisor for Registered Practitioners in the team.

You will be working flexibly across a wide geographical area including from home, so the ability to travel in a timely manner according to business need is essential.

This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English sufficient for the effective performance of the job requirements. This post is subject to an Enhanced Disclosure application to the Disclosure and Barring service and registration as a qualified Social Worker with Social Work England.
