As a Case Officer within the Short‑Term Pathways Team, you will be working with people over the age of 18 with a wide range of needs. You will manage a small caseload requiring assessment, review, and the arrangement of on‑going support to make a real difference in achieving improved outcomes and promoting independence.
Focusing on planning ongoing care and support for adults who have recently had a hospital admission, this role is within a team that works collaboratively with health colleagues and other partnership agencies and focuses on promoting a person’s independence.
Whilst mainly working from home at the current time, a fixed working base will be discussed at interview. The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential – this covers the whole of West Kent area.
The post is considered by KCC to be a customer‑facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post‑holders have a command of spoken English sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification. The post is subject to an Enhanced Disclosure application to the Disclosure and Barring Service.
