Are you a confident, proactive, and positive individual looking to make a difference in Adult Social Care? A fantastic opportunity has arisen within our Arranging Support Hospital Discharge Team, supporting older people and those with physical disabilities to return home safely from hospital.
This role is ideal for someone with a strong background in administration, customer service, purchasing, or procurement, who is keen to develop their skills in the social care sector.
Key Responsibilities:
- Arrange domiciliary care packages to support hospital discharge
- Arrange support in line with Council contracts and purchasing protocols.
- Communicate effectively with care agencies and other professionals
- Maintain accurate records, update client systems and other IT tools.
- Investigate and resolve payment queries.
- Handle sensitive and confidential information appropriately.
