We are looking for an enthusiastic and adaptable Urgent Care Service Nurse to join our strong and progressive team in Folkestone. You will share the CARE values of the trust and work within a close-knit group of people who bring the feeling of community to the team through their hard work and dedication to the people of the area.
The Urgent Care Service in Kent is encouraging innovative change to ensure that patient care is holistic and needs-led. We are a consultant led team of Doctors, Advanced Clinical Practitioners, Occupational Therapist, Physiotherapist, Nurses and Health Care Assistants specialised in delivering short term hospital level care to people in the home environment. Community is the heart of our trust and we are working with primary care colleagues locally to respond to the needs of our patients and we want you to be part of this change.
We aim to respond to referrals within 2 hours and will arrange to visit to assess, diagnose, treat and implement care strategies that will enable our patients to remain safe and well in the comfort of their own home. Folkestone is made up of a diverse patient group with complex needs and by working in a community-based way we can ensure that services are targeted and patient-focused, and we are proactive in our input and response.
Main duties of the job
You will play a vital role in our multidisciplinary services; managing patients with complex needs in the community. You will do this by completing comprehensive frailty assessments and working with our patients and their families to build clear plans based on current healthcare needs but also the wishes and preferences for future care.
You’ll have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, and access to structured supervision and a clinical lead to make sure you are received the support necessary.
