The Rough Sleeper Nursing Service, run by Kent Community Health NHS Foundation Trust, offers specialist outreach care for people rough sleeping or living in unstable housing across East Kent, West Kent, and Medway. This service provides trauma-informed healthcare in accessible locations like hostels, day centres, temporary accommodation, and even on the streets. Nurses offer wound care, manage long-term conditions, conduct physical health checks, give harm reduction advice, and promote health to enhance wellbeing and reduce hospital admissions.
The Rough Sleeper Nurse is essential in providing specialized, trauma-informed healthcare to individuals sleeping rough or staying in temporary housing throughout the Medway area. As part of Kent Community Health NHS Foundation Trust\’s Rough Sleeper Nursing Service, this role offers flexible, community-based support such as wound care, managing long-term conditions, physical health checks, and promoting overall health.
Additional Working Pattern: Monday – Friday 9-5
