Job overview
To work as a member of the Home Treatment and Rapid Response services, delivering support to patients and their families in the home environment, using evidence-based, profession-specific, and general mental health knowledge. The role involves providing client-centered, recovery-oriented care for individuals, families, and carers referred to the service. You will also undertake assessments 24/7 from the Urgent Mental Health Helpline and conduct gatekeeping assessments, working in accordance with Trust and Crisis Resolution Home Treatment Team policies and the Care Programme Approach. The aim is to provide an alternative to inpatient admission by treating patients at home during crises.
Service areas include Dover/Deal, Shepway, and Ashford.
Main duties of the job
- Contribute to the assessment of clients referred to Home Treatment and Rapid Response
- Support clients and carers in personal care, daily living, social activities, and maintaining social networks through various home treatment interventions
- Carry out specified interventions independently as per crisis plans and update as needed
- Monitor client progress against crisis plans, communicate findings, and suggest adjustments to senior staff for review
- Report changes in client presentation and safety concerns to senior staff
- Respond flexibly to crises, adjusting work priorities as required by the Shift Co-Ordinator
- Engage clients and carers with crisis plans, promoting crisis prevention, recovery, and resilience
- Support carers to help clients remain at home, and assist with medication administration as per crisis plans
- Promote the work of Home Treatment and Rapid Response to clients, carers, and other professionals and agencies
