Purpose of the Post:
To support the Care Services team in delivering high-quality, holistic care to patients, families, and carers across our Day Hospice and Hospice at Home services. The Health Care Support Worker plays an important role in providing personal, emotional, and psychological support, carrying out delegated clinical tasks in line with competencies, and helping to ensure every person receives compassionate, person-centred care in line with KEMP Hospice’s values.
Main duties and responsibilities:
Clinical Responsibilities:- Provide personal care including washing, dressing, toileting, and mobility support, always ensuring dignity, comfort, and respect for the individual.
- Support patients to maintain independence, choice, and control over their care, delivering person-centred support that reflects individual needs and wishes.
- Assist with eating, drinking, and medication prompts as directed, always ensuring safe and supportive care.
- Observe and promptly report any changes in patient condition to the appropriate member of the clinical team.
- Follow infection prevention and control procedures consistently to maintain a safe and hygienic environment.
- Deliver care within both Day Hospice and Hospice at Home services, supporting group activities, therapies, respite, and end-of-life care visits.
- Record all care given accurately and promptly using electronic patient records.
- Contribute to multidisciplinary team (MDT) discussions and care planning to ensure holistic support for patients and families.
- Promote dignity, compassion, and choice in all aspects of care, supporting advanced care planning in line with patient preferences.
- Work collaboratively with nursing colleagues, therapists, volunteers, and the wider Care Services team to provide seamless, high-quality care.
- Communicate clearly, compassionately, and sensitively with patients, families, carers, and colleagues.
- Support volunteers and new team members, helping them feel included and confident in their roles.
- Share information appropriately within the team to maintain continuity and safety of care.
- Demonstrate a positive, flexible approach, responding to the changing needs of the service and supporting colleagues when required.
- Take part in audits, reviews, and infection control activities to maintain high clinical standards.
- Maintain awareness of CQC requirements and contribute to the delivery of safe, effective, and responsive care.

- Care Certificate or Level 2 or 3 qualification in Health and Social Care (or equivalent).
- Other relevant health or social care qualifications.
- Recent palliative care or end-of-life care education.
- Recent training in patient moving and handling procedures.
- Experience of working as a HCA/Health Care Support Worker or similar role in the healthcare, community or social care setting.
- Awareness of the bereavement process.
- Experience of supporting patients with palliative and/or end-of-life needs.
- Experience in community or hospice services.
- Familiarity with virtual platforms such as Teams/Zoom.
- Excellent communication skills including verbal, written and presentation.
- Ability to carry out basic clinical observations.
- Safe moving and handling techniques.
- Demonstrate understanding of infection control requirements.
- Ability to work autonomously and as part of a team.
- Highly motivated and enthusiastic.
