We are looking for a passionate Children's Palliative Care Support Worker to join our dedicated community team and provide high quality individual support to a child and their family both at their home and in the hospice. You will provide comfort and care to support our younger patients and make a difference when it matters the most.
Key Responsibilities:
- Provide patient centred personal and clinical care and support of children and young people with complex needs in accordance with the care plan in a range of settings.
- Deliver a high standard of specialist palliative care in partnership with other professionals.
- Provide an empathetic and supportive presence for families and refer to appropriate services as needed.
- Act as an advocate for children and their families.
- Engage the patient with a range of activities that meets their needs.
What we are looking for:
- Previous work or personal experience in caring for children with complex health needs.
- Excellent verbal and written communication skills.
- Strong understanding of safeguarding children.
- Ability to work unsupervised with shifting priorities and organise own workload.
- NVQ Trained to level 2.
- Full UK driving licence.
