Community Mental Health Practitioner / OT – Band 6
Location: Witney
Salary: £35.00 – 45.00 per hour

The CMHT work primarily with working age adults who have a diagnosis of a Serious Mental Illness and/or Complex Mental Health Difficulties. The care pathway includes a holistic assessment of needs and risks, formulation, and the delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning.
The practitioner will also:
- Work collaboratively across a range of disciplines to promote a ‘needs-based’ approach to all service users.
- Promote an integrated approach to service delivery.
- Develop clear relapse recognition and prevention plans and share these with MDT, service users, and carers where appropriate.
- Develop meaningful engagement strategies with individuals who are often referred to as ‘difficult to engage’ and their families.
