Community Mental Health Practitioner / OT – Band 6

Location: Witney

Salary: £35.00 – 45.00 per hour

Working hours: Part Time

The CMHT work primarily with working age adults who have a diagnosis of a Serious Mental Illness and/or Complex Mental Health Difficulties. The care pathway includes a holistic assessment of needs and risks, formulation, and the delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning.

The practitioner will also:

  1. Work collaboratively across a range of disciplines to promote a ‘needs-based’ approach to all service users.
  2. Promote an integrated approach to service delivery.
  3. Develop clear relapse recognition and prevention plans and share these with MDT, service users, and carers where appropriate.
  4. Develop meaningful engagement strategies with individuals who are often referred to as ‘difficult to engage’ and their families.
