The focus of your role is to assess, facilitate, and provide lifesaving treatment in a timely manner, ensuring prompt triage for staff to deliver immediate hyper-acute care for the first 72 hours post-stroke.

As a Band 6 Specialist Stroke Nurse, you will be required to provide specialist support, advice, and training in stroke care. You will work at a specialist clinical level as an autonomous practitioner to deliver community-based, person-centered care and ongoing advice for adults who have had a stroke, using evidence-based interventions. You will be responsible for your own clinical caseload, working independently to deliver advice, support, and monitoring of service users who have experienced a stroke.

You will be involved at every stage of the patient journey within the Stroke Service to support patients and facilitate a seamless transition of care from the Emergency Department to the Stroke Unit, and through liaison with community teams at discharge. You will work closely with all relevant departments and colleagues across the multidisciplinary team to achieve coordinated care that meets national and local guidelines. The team provides a point of contact and expertise in stroke specialty to staff across the organization, as well as to patients and their families/carers.

The Stroke Practitioner Service receives direct referrals within the specialty and is able to provide expert assessment and evaluation of patients\’ needs. A key aspect of the role is to provide education, training, and support to staff, patients, and their families.
