Due to internal promotion we are looking for a Clinical Pharmacist to join the team. As a Clinical Pharmacist at K2 Healthcare Ltd, you will work with our established Clinical Pharmacist Team to develop and deliver medicines optimisation services within our group of GP Practices. This is an exciting opportunity to join our federation and collaborate with Senior Clinical Pharmacists, Clinical Pharmacists and Pharmacy Technicians as part of a multidisciplinary team.
This is a patient‑facing role requiring clinical assessment and treatment using specialist medicines knowledge across specific disease areas. You will be registered with the GPhC or about to complete your foundation training in order to register as a Clinical Pharmacist with the GPhC. You may already be an independent prescriber or working towards this qualification. The role involves proactively supporting patients with complex polypharmacy, particularly older adults and those with multiple long‑term conditions. You will have opportunities to develop clinical interests, enhance prescribing skills, and improve patient outcomes.

- Promoting safer prescribing systems
- Working within a supported team environment with mentoring and guidance
- Participating in up to 28 study days over the first 18 months of professional development
Qualifications
- Registered with the GPhC or in the final year of GPhC foundation training
- Independent prescriber or in progress to become one (if applicable)
- Experience with complex polypharmacy in older adults and patients with multiple long‑term conditions
K2 Healthcare is a GP Federation covering 16 practices (10 in Grantham and surrounding villages, 6 in and around Sleaford) that serve 130,000+ patients across all sites. The Pharmacy Team consists of 3 Senior Clinical Pharmacists, 12 Clinical Pharmacists and 7 Pharmacy Technicians. The Clinical Pharmacy Team are at different stages of completing their CPPE / IP course and are happy to share their experience with you.
