As a Clinical Pharmacist, you will work with our established Clinical Pharmacist Team to develop and deliver medicines optimisation services within our practice. This is an exciting opportunity to join our federation and collaborate with Senior Clinical Pharmacists as part of a multidisciplinary team. Our current team includes 3 Senior Clinical Pharmacists, 12 Clinical Pharmacists, and 7 Pharmacy Technicians. We are seeking a motivated individual to support the continued development of our Pharmacy Team.
This is a patient-facing role requiring clinical assessment and treatment using specialist medicines knowledge across specific disease areas. You may already be an independent prescriber or working towards this qualification. The role involves proactively supporting patients with complex polypharmacy, particularly older adults and those with multiple long-term conditions. You will have opportunities to develop clinical interests, enhance prescribing skills, and improve patient outcomes.
Working across a small number of practices will allow you to build strong relationships within primary care teams. Responsibilities include managing repeat prescription authorisation, acute requests, transfer of care processes, and promoting safer prescribing systems. Ongoing professional development is supported through a clinical network, including 28 study days over the first 18 months. You will always work within a supported team environment, with mentoring and guidance provided.
Main duties of the jobYou will provide primary support to general practice staff with regards to prescription and medication queries. You will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).
You will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. You will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. You will work with the other Clinical Pharmacists in our large team being fully supported throughout.

We have a Pharmacy Team that consists of 3 Senior Clinical Pharmacists, 12 Clinical Pharmacists and 7 Pharmacy Technicians. Our Clinical Pharmacy Team are all at different stages of completing their CPPE / IP course and are happy to speak to you regarding their experience with K2 Healthcare.
Job responsibilitiesThe post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside the Pharmacist and Dispensary team in the Practice, who will help develop, manage and mentor them. The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

Person SpecificationQualifications
Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council. Hold or be working towards an independent prescribing qualification. Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate. Completed CPPE Pathway.
Experience
Experience of working in Primary Care.

£45,000 to £46,000 a year (£45K on starting, rising to £46k when IP and CPPE completed).
