As the Team physiotherapist , you will be supporting the multi-disciplinary team and people who will be, and have been, discharged from hospital. You will liaise with individuals, their families, and other Health & Social Care professionals to ensure that the necessary actions and messages are relayed to enable effective short-term care. You will need to be an excellent communicator who enjoys teamwork and thrives in a fast-paced environment.
The Physiotherapist, will act as the source of expertise on the assessment and management of residents presenting with a wide range of pathologies to physiotherapists, skilled non registered staff, other healthcare professionals, GP’s, the residents, social care colleagues and their carers. You are pivotal to the day-to-day delivery of the service and will have regular contact with the patients and their families. Key Core Hours – 08.00 – 17.00 = 37.5 hrs per week Monday – Friday:
To enable delivery, you must be able to do the following:

Demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

Attend the twice-weekly meeting and record actions and notes accordingly.

Provide physiotherapy expertise within the multi-disciplinary team to people discharged with Home First.

Complete face to face assessments with people in the community or a temporary placement who require therapy input.

Liaise and ensure that any on-going Physiotherapy needs are transferred to reablement (STS&R) or Community Physios.

Lead on Day 1 visits where appropriate, complete assessments, identify goals and a rehabilitation plan.

Manage a varied caseload, spanning all physiotherapy specialities.

Co-ordinate discharge and therapy interventions for people leaving temporary placements.

Provide information to individuals and their families about their condition and how to manage it effectively, ie, exercise programmes.

Liaise with the remote monitoring team where an alert has been raised, to address concerns.

Following therapy input, identify potential on-going care needs and solutions to maximize independence.

To retain professional records in line with your organisation requirements and to complete the Home First records.

Contribute to the twice-weekly case meeting, providing information to the team about a persons condition, treatment plan and progress.

Advocate for people who require additional health services, ie Continuing Health Care referrals, hospice care and other community health services

Completion of pre-discharge assessment for people waiting to come out of temporary placements onto Home First.

Liaise with staff in the hospital to co-ordinate and manage discharge for people with complex needs.
