JustOT is currently recruiting for an experienced Band 8a Complex Care Programme Case Manager to join a service in South East London on a locum basis.
This is an excellent opportunity for an experienced professional to play a key role in supporting complex care pathways, working with individuals across Learning Disabilities, Autism and Older People's services.
The successful candidate will have strong clinical and leadership experience, with the ability to manage complex cases, coordinate multi-agency support and ensure individuals receive appropriate, person-centred care.
Key Responsibilities:
- Manage complex care cases across learning disabilities, autism and older people's services.
- Coordinate care planning and ensure effective management of complex needs.
- Work collaboratively with multidisciplinary teams and external agencies.
- Support the development and delivery of complex care programmes.
- Review care pathways and identify opportunities to improve outcomes and service delivery.
- Provide specialist advice, guidance and support to colleagues and partner services.
The Ideal Candidate:
- Previous experience in a Band 8a or equivalent senior complex care role.
- Strong knowledge of learning disabilities, autism and older people's services.
- Experience managing complex cases and coordinating multi-agency care.
- Excellent communication, leadership and organisational skills.
- Ability to work autonomously and make informed decisions.
