The successful candidate will work closely with service users, the Therapy Team, Rehabilitation Coaches, and wider multidisciplinary professionals to deliver high-quality, person-centred rehabilitation programmes that promote independence, wellbeing, and positive outcomes.

Key Responsibilities

  • Lead and coordinate all aspects of client care relevant to the assigned pathway.
  • Support the Interdisciplinary Team (IDT) by interpreting assessments and contributing recommendations to the wider rehabilitation process.
  • Work collaboratively with internal and external therapists, stakeholders, and professionals to ensure the highest standards of client-centred care.
  • Plan, implement, and review therapeutic interventions that enhance wellbeing and rehabilitation outcomes.
  • Maximise functional ability, reduce dependency on services, and support individuals to achieve greater independence.
  • Complete admission, discharge, and periodic assessments, including MDT transfer meetings.
  • Set agreed rehabilitation goals, monitor progress, and take responsibility for key rehabilitation KPIs linked to client progression, wellbeing, and engagement.
  • Provide training and guidance to service users and staff to promote functional independence.
  • Work closely with the Lead Neuro Consultant to deliver induction, routine, and ad hoc staff training.
  • Advise on equipment, environmental adaptations, and modifications to support daily living.
  • Develop strong community links to enable timely access to external resources and adaptations.
  • Maintain accurate case records and prepare reports to evidence progress and support recommendations for ongoing care or referral.
  • Liaise with Local Authorities, external agencies, and healthcare partners as required.
  • Contribute to service development projects, care pathway improvements, and measurable outcome reporting.
  • Support Integrated Care Plans (ICPs) and transdisciplinary working across the service.
  • Attend meetings relating to service planning, development, and quality improvement.
  • Build external partnerships, community links, and professional networks.

Requirements

  • Degree in Occupational Therapy or equivalent professional qualification.
  • Current HCPC Registration.
  • Relevant post-qualification experience, ideally within Brain Injury / Neuro Rehabilitation Services.
  • Experience working at senior level within Health & Social Care settings.
  • Strong understanding of person-centred support planning.
  • Ability to manage and prioritise a varied workload effectively.
  • Excellent communication and stakeholder management skills.
  • Ability to influence commissioners, service users, and clinical/non-clinical stakeholders.
  • Knowledge of regulatory and statutory frameworks relevant to health and social care services.
  • Experience managing budgets and delivering service objectives would be advantageous.

Our Client Offers

  • Fantastic career opportunity
  • Competitive salary
  • Supportive and highly skilled team environment
