We are looking for a proactive and digitally confident Digital Care Coordinator to join Jurassic Coast Primary Care Network, supporting three local practices: Ammonite Health Partnership, Barton House Medical Practice and Lyme Bay Medical Practice. This is an exciting opportunity to play a key role in how healthcare is evolving locally. As digital tools, data and new technologies become central to modern healthcare, this role will help both patients and healthcare professionals make the most of these opportunities.
You will work closely with clinicians, administrators and PCN leadership to support the use of digital systems, population health data and innovative technologies such as healthcare apps, remote monitoring and shared care records. Your work will help identify patients who may benefit from proactive support, improve access to services and support the development of new ways of working across the network. This role is ideal for someone who enjoys working with digital systems, data and people. You will help translate data and technology into practical action, supporting colleagues and patients to feel confident using digital healthcare tools.

Main duties of the job
The Digital Care Coordinator will support the development and delivery of digitally enabled care across Jurassic Coast Primary Care Network. The role focuses on helping both patients and staff make effective use of digital tools, data and systems to improve access to services and support better health outcomes.
The postholder will work closely with clinicians, administrators and PCN leadership to use population health data and clinical systems to identify patients who may benefit from proactive support or new services. They will also assist practices in embedding digital solutions into everyday workflows, ensuring systems are used efficiently and consistently across the network.
The role includes supporting colleagues with digital systems, helping maintain accurate datasets, assisting with service monitoring and providing practical support to PCN projects and service developments. The Digital Care Coordinator will also contribute to improving digital engagement by supporting communication through websites, social media and patient-facing digital resources.
This is a varied role that combines digital systems, data insight, administration and collaboration with a wide range of healthcare professionals to support the delivery of modern, patient-centred care across the PCN.

Job responsibilities
- Digital Leadership and Innovation
- Act as a champion for digital transformation across the PCN, promoting the effective use of digital tools, systems and new ways of working.
- Support the implementation of digital healthcare technologies including healthcare apps, remote monitoring tools and shared care records.
- Work with PCN leadership to support the delivery of digital elements within PCN service development and transformation programmes.
- Lead on the practical deployment of new digital tools, ensuring both staff and patients are supported to use them effectively.
- Population Health and Data Insight
- Run proactive searches and population health reports using SystmOne and other systems to identify patients who may benefit from PCN services and targeted interventions.
- Use population health data, Dorset Intelligence and Insight Service tools and local datasets to support proactive care and service planning.
- Analyse service uptake and digital engagement data to support service improvement and PCN planning.
- Maintain accurate datasets and support data quality processes across digital and care coordination systems.
- Operational and Administrative Support
- Work closely with administrative and clinical teams to integrate digital systems into day-to-day workflows.
- Support the development and maintenance of system templates, rotas, searches and operational set-up within SystmOne and other systems.
- Assist with pathway administration and coordination linked to PCN projects and services.
- Ensure digital processes support efficient, consistent and high-quality service delivery across the network.
- Education and Support
- Act as a first point of contact for digital queries from colleagues and patients.
- Provide training and guidance to colleagues to improve confidence in using digital tools and data systems.
- Support patients to access and use digital health technologies where appropriate.
- Troubleshoot basic IT, software and system issues relating to digital tools used across the PCN.

- Support the management of PCN digital communications including websites, social media and digital campaigns.

- Provide operational and digital support to PCN projects and service developments.

Person Specification
Skills and Knowledge
- Understanding of the personalised care approach and how it supports patient engagement and self-management.
- Knowledge of SystmOne or similar clinical systems.
- Competent in Microsoft Office packages including Word, Excel, Outlook and PowerPoint.
- Experience managing datasets, maintaining data quality and producing routine reports.
- Ability to manage digital communications including website updates or social media content where required.
- Good working knowledge of IT systems, software and basic hardware.
- Experience producing reports within SystmOne (S1 reporting).
- Experience managing rotas or templates within clinical systems.
- Advanced Excel skills including pivot tables or macros.
- Awareness of NHS digital strategy and digital transformation in primary care.
- Knowledge of Dorset Intelligence and Insight Service (population health management and stratification tools).
- Ability to interpret data, identify key themes and present findings clearly.
Experience
- Experience working with healthcare professionals, or within NHS, social care or a related sector (paid or voluntary).
