Julia's House is seeking a Band 6 Transition Nurse to support children and young people with life-limiting conditions. You will coordinate transition to adulthood, maintain continuity of care, and work across hospice, home and community settings with a dedicated caseload and strong family partnerships.
You will be registered nurses with NMC, experienced in pediatrics and palliative care, and flexible to cover varied shifts to meet family needs. Relocation support may be considered.
