The Social Worker provides quality psychosocial care and coordinates with the medical team to address patients’ needs. This role involves assessing patients’ social and emotional factors affecting their medical care, implementing interventions, and connecting families with resources. The Social Worker plays a vital role in the healthcare team while adhering to JHACH policies and regulations.

Responsibilities

  • Conduct psychosocial assessments to identify and address patients’ social and emotional needs impacting medical treatment.
  • Perform suicide risk assessments and recommend safety measures to the medical team.
  • Refer patients to multidisciplinary team members and community resources for coordinated care.
  • Deliver therapeutic interventions, including crisis intervention, grief counseling, and psychoeducation.
  • Collaborate with the medical team on care plans, identifying psychosocial barriers to discharge.
  • Maintain timely electronic psychosocial documentation and record-keeping.
  • Communicate effectively with team members about patient needs and participate in team meetings and rounds.

Qualifications

  • Master’s Degree in Social Work.
  • Completion of JHACH’s intern program or a comparable external healthcare internship.
  • Strong listening and communication skills.
  • Proficient documentation skills in electronic medical records.
