Provides individualized care for all aspects of medical nutrition therapy (MNT) and diabetes self‑management including meal planning, diabetes medications, exercise, how to check blood glucose and how to integrate diabetes care into everyday life. Works collaboratively with a multi‑disciplinary health team to promote wellness, maintain current health, and/or intervene in acute or chronic illness in the pediatric population. Utilizes the Nutrition Care Process to provide quality care to patients. Acts within the scope of practice and is responsible for patient outcomes that meet the high standards of care provided by JHACH. Contributes to the promotion of a culture of innovation and a commitment to academic growth and professional development.
Key Accountabilities
- Educates patients and families on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.
- Plans, coordinates, implements and evaluates nutritional interventions based on the assessment of the patient/family to achieve progress toward desired clinical outcomes.
- Documents expected outcomes or measurable goals, interventions performed and response to assist clinical practitioners including physicians in care coordination.
- Collects patient data to monitor health status and proactively manage the patient’s condition.
- Collaborates with other health care professionals to provide diabetes self‑management education (DSME) and diabetes self‑management support (DSMS) to individuals and groups.
- Facilitates learning using education to close knowledge gaps.
- Develops data driven educational programs and learning opportunities.
- Utilizes evaluation methods to measure and report outcomes.
Qualifications
- Bachelor’s degree in related field from an accredited college or university.
- Master’s degree from an accredited college or university, preferred.
- Registered Dietitian/Nutritionist by the Commission on Dietetic Registration, Academy of Nutrition and Dietetics.
- Dietetics/Nutritionist Licensure in the State of Florida.
- Certified Diabetes Care and Education Specialist (CDCES) credential required within one (1) year of hire.
- At least three (3) years in a clinical setting required, with at least one (1) year of pediatric experience preferred.
- Ability to interact with patients and family members in a positive manner to teach diabetes principles.
Salary Range: Minimum 33.70/hour - Maximum 53.93/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
