The Oncology Unit Based Pharmacist is a patient-centered, unit-based model of care that integrates pharmacists with responsibility for both the clinical and the distributive activities of the pharmacy department. Pharmacists are actively engaged in medication management with strong focus on areas such as medication utilization assessment, pain management and antimicrobial stewardship, and exhibit a high degree of ownership and accountability for the medication-use process. The Oncology Unit Based pharmacist, in collaboration with physicians and the interdisciplinary team, supports the multiple oncology practices within the institution.

Responsibilities:

  • Validate medication reconciliation: Reconcile medications upon admission for admitted patients. Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care.
  • Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients. Educate patients about new medications. Counsel patients who are prescribed high-risk medications or complex medication regimens on their discharge medications during inpatient hospitalization.
  • Facilitate discharges: Identify barriers to patient medication adherence and work with the interdisciplinary team to address these issues prior to discharge.
  • Collaborate with the doctors to optimize cost-effective and evidence based: Review and validate all orders prior to the first dose administered. Monitor patient medication profiles daily. Participate in patient care rounds.
  • Drive improved outcomes: Prevent 30 day readmissions, increase HCAHPS scores, and improve patient safety indicators.
  • Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications:

  • Education, Knowledge, Skills and Abilities Required: Bachelor’s Degree from an accredited college of Pharmacy. Possess strong interpersonal skills and work well at the team level, as well as independently. Excellent written and verbal communication skills. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
  • Licenses And Certifications Required: Registered Pharmacist License. Advanced Cardiac Life Support Certification within 6 months of entering position.

Compensation: Minimum rate of $146,057.60 Annually. HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.
