  • Undertake assessments and support planning for adults wishing to move to Housing with Care or Independent Living schemes
  • Work with providers and landlords throughout the nomination and moving period
  • Support smooth transitions into Housing with Care and Independent Living
  • Complete initial reviews after individuals move into schemes
  • Act as link worker for Housing with Care and Independent Living schemes within Norwich
  • Complete reassessments and reviews in response to changing needs and circumstances
  • Promote wellbeing and independence
  • Work with adults in other settings when required
  • Collaborate with Social Workers, Occupational Therapists, Assistant Practitioners, providers and landlords
  • Contribute to strengths-based, preventative and community-led approaches within Adult Social Care

Requirements

  • Passion for supporting people to remain as independent as possible
  • Skilled communication and ability to build effective, trusting relationships
  • Creative, curious and solution-focused approach
  • Organisation and ability to manage competing priorities
  • Confidence using IT systems and electronic recording systems
  • Commitment to high-quality, person-centred practice
  • Knowledge and understanding of the Care Act 2014
  • Knowledge and understanding of Community Led Support
  • Knowledge and understanding of the Mental Capacity Act 2005
  • Knowledge and understanding of Safeguarding Adults
  • Knowledge and understanding of strengths-based practice
  • Knowledge and understanding of person-centred approaches
  • Knowledge and understanding of promoting independence and wellbeing
  • Knowledge and understanding of partnership and multi-agency working
  • CV and Supporting Statement required
  • Supported Assistant Practitioners join a 9-month development programme and progress subject to satisfactory completion
