- Work within Adult Social Care’s Out of County Hospital Team covering Lewes and North Wealden
- Occasionally undertake work for the Community Hospital Social Work Team covering Eastbourne and Hastings
- Collaborate with local partners and hospital trusts to support safe discharge from hospital to people’s homes
- Promote the home first model and discharge to assess approach
- Support clients to maintain independence and remain safely at home where possible
- Work with reablement teams and inpatient rehabilitation providers to maximise client independence and build confidence
- Work with local Integrated Commissioning Boards to help people access continuing care funding
- Travel across East Sussex, including rural areas
- Participate in interview panel process if applicable
Requirements
- Qualified Social Worker
- Registered Social Worker with Social Work England
- Social work qualification
- Completed Assessed and Supported Year in Employment or equivalent if prior to 2012 (NQSW year and academic module)
- Ability to meet travel requirements across East Sussex, including rural areas
- Resilience and flexibility to manage a dynamic and complex caseload
- Knowledge of appropriate legal and policy frameworks informing and mandating social work practice
- Enhanced DBS check or Enhanced check for Regulated Activity required
- Evidence of an overseas police check if lived or worked abroad
- Applicants requiring UK work sponsorship must disclose this so eligibility can be checked
