- Work with families, schools and professionals to gather information for Education, Health and Care Needs Assessment requests
- Collate and organise information from multiple sources for presentation to a multi-disciplinary panel
- Build positive relationships with parents, carers, educational settings, health colleagues and other stakeholders
- Act as a key contact, respond to enquiries and provide updates throughout the assessment request process
- Maintain accurate records using data recording and case management systems
- Monitor and track requests to ensure processes progress within required timescales
- Collaborate with colleagues across the SEND service to support effective decision-making and quality outcomes
- Identify opportunities to improve processes and the experience of families and professionals
Requirements
- Strong organisational skills and ability to manage and prioritise a varied workload
- Excellent communication skills and ability to build positive relationships with families, schools, professionals and colleagues
- Ability to gather, collate and present information clearly and accurately
- Keen eye for detail and commitment to maintaining accurate records across multiple systems
- Confidence using IT systems and databases, with ability to learn new systems and processes
- Collaborative approach and ability to work as part of a supportive team
- Ability to handle sensitive information with professionalism, discretion and empathy

- Understanding of the needs of children and young people with SEND and their families

- DBS checks or police vetting required for relevant posts
