Job Responsibilities:
- Conduct intake assessments and participate in case conferences with multidisciplinary teams.
- Collaborate with healthcare professionals to develop and review Individual Care Plans (ICPs).
- Monitor clients' progress and evaluate care outcomes.
- Conduct home and hospital visits when required.
- Provide guidance on relevant financial assistance schemes and community resources.
- Perform other duties as assigned by the reporting manager.
Requirements:
- Degree or Graduate Diploma in Social Work.
- Relevant experience in healthcare or community settings will be an advantage.
